As the new NHS long-term plan [Jan. 2019] says, “Patients Personal Health Records will hold a care plan that includes information added by the patient, or their home carers. Making personal care plans available to the patient, and all clinicians caring for them, will help ensure the right care actions are taken in good time” [para. 5.15].

Care planning is discussions with you, your home carer(s), your doctor and other health specialists you use. Together, look at all your needs with you, to ensure you get the right care. e.g . check heart health, or how to prevent diabetes. As the UK NHS says, “Everyone with a long-term condition should have a written healthcare plan, if they want one”.
Write your own personal Health Plan to enjoy your health for 6 to 9 years longer.
Set out in writing your own health needs, health outcomes/targets you want to achieve, and how you will achieve them. So you and your home carer will not forget the details!
If you have health worries or questions, note these in your plan to discuss with your doctors.
If you put dates, and how often you will do them, on actions in your plan then you can use your mobile phone, tablet or PC to remind you of these actions and dates.
In 2017, 85-89 year olds, now three times as many as there were in 1971, had twice the rate of ill health than those aged 50-54, e.g. high blood pressure, diabetes, heart diseases, strokes. The gap in life expectancy between those living in the least and most deprived areas of the country was 9.4 years for males and 7.4 years for females. People in the least deprived areas of the country enjoy 19 more years in good health [1]. Cut these health gaps with your personal health plan.
There is a big change, just starting, in how patient, home carer and doctors work together. Soon it will become our (digital) data, and our duty to take action before we get seriously ill — not after. Some people are already self-managing as part of the future of healthcare. Self-tracking is here to stay e.g. Expect to see a blood pressure monitor in the nation’s bathroom cabinet. “This is a no-brainer,” Dr Michael Snyder at Stanford University USA, tells us, “Everyone already does home shopping. Why would we not do home healthcare? Why wait to go to a doctor’s office where you can get a virus?”

3.1. Suggest you create a new file on your smart mobile, desktop PC or tablet, and name it, e.g. ‘My health plan’. Then start copying the headings and suggested details that are important to you, from the ones listed on ourhealth plan make-up page. This gives you a framework for your plan.
3.2. Then, as you read these and other health web pages and trusted health magazines, e.g. https://thrive-magazine.co.uk/, with your home carer you can decide on your actions, and begin to put them in your plan under those headings.
